Anal fissures are sometimes mistaken for hemorrhoids. They are small splits or cracks in the skin near the anus, often caused by large, hard bowel movements. Moderate to severe itching in and around the anus (pruritus ani) also feels like hemorrhoids. Anal itching can also be due to other conditions such as heat and tight-fitting clothing, improper hygiene, dry skin, excessive washing and wiping, chemical and food irritants, anal tumors or stress.
